Exosome Therapy: Aesthetic Syracuse

This treatment is a new evolution. Exosome is the extension a new paradigm, focusing on natural improvement and regeneration in your life. Exosomes build on the experience with injections of chorionic tissue. Exosome therapy can address both cosmetic and functional concerns. The early experience has been encouraging, but treatment parameters and results are still evolving.

Exosomes: What are they?

Exosomes are created by stem cells. They represent the “business” part of a stem cell. It is a “message in a bottle,” transferring information to other cells. For these treatments, stem cells are harvested from placental tissue donated from c-sections. The cells are cultured, with the exosomes collected and processed. Each milliliter of the material I use contains approximately 10 billion exosomes. This is the equivalent of about 10 million stem cells.

The treatment

To date, treatment has been by local or intravenous injection, using 1 cc, 2 cc, or 5 cc of exosomes. Like the chorion injections used for the injectable facelift, the exosomes stimulate repair, but more rapidly than the chorion injection. Due to their microscopic size, exosomes can be injected using the same tiny needle used for insulin. In the near future, there may be a topical form for skin repair.

Facial Rejuvenation: Treatment can be superficial, deep, or both. For comfort, treatment is usually done using anesthetic similar to going to the dentist. Due to the final volume used, you can expect some puffiness, and possible bruising. Most patients feel comfortable going out the next day.

I consider deep treatment in areas of higher dynamic activity: Your nasolabial folds (connecting your nose-to-the-corner of your mouth-to-your chin), deep lip lines, glabellar lines (“The 11’s”), crow’s feet and the hollow beneath your eyes and cheek. Placed beneath your skin, my goal is to stimulate restoration and repair of the network of blood vessels and the supporting soft tissues of your cheeks.

Superficial treatment targets specific areas, such as crow’s feet, lip lines, or acne scars. Placed in your skin my goal is to stimulate repair of the skin with a more permanent, dynamic skin quality. It may also play a role in treating acute inflammatory acne outbreaks associated with deep scars. There is a synergy between exosome injection and argon plasma skin treatment.

In combination with other treatment, the goal is to augment the results of treatment. This is helpful in two ways: For grafting procedures, such as fat injection, or to help limit scarring either in sensitive patients or in those with a history of keloid.

For fat graft/injection patients, this treatment may be used at three times: Before, during, or after fat injection. Of the three, I believe that pre-treatment may provide the most benefit. Pre-treatment will help increase the blood supply (angiogenesis) and the available stem cells for healing. This can create a “more responsive” recipient site. During and after fat injection, the effects can be the same, but in terms of the biggest anticipated “bang for your buck,” I believe pre-treatment is preferred.

Results: Improvement occurs over time, with final results at 1 year. You continue to age, so maintenance treatments may be recommended. While the changes may be subtle in photographs, the dynamic result, which is the true reflection of youth, is more pronounced. Because it is a natural change, you are more likely to have friends and relatives, comment “You look good!” But without quite being able to identify what is different. By example, several patients have commented that their spouse was unsure about their treatment, but that they were now pleased with the results because they look much better, but did not look different.

Skin Rejuvenation Elsewhere: Like facial rejuvenation, exosomes can help with superficial or deep treatment. While it is not yet a cure, therapy has been helpful for lax tissues, like baggy knees, aging hands, and crepe skin of the inner arms. Most treatments work best starting with deeper tissue rejuvenation.

Hair Loss: Exosomes injections have shown great promise, even in those that are bald. Hair growth has been seen in bald patients. Hair growth is typically seen within 8-10 weeks

General Health: Exosomes have been administered intravenously for general health reasons. The single biggest response has been improved energy levels, with increased exercise tolerance. Improved skin quality was also noted for the crepe inner arms, abdominal contour, and above the knees

Aside from the risk with any injection, the effect of exosomes on tumor cells is unknown. Equal arguments can be made for exosomes stimulating cancer, suppressing cancer, or having no effect. With local injections, unless there are tumor cells present in the area, I believe the risk is small. For someone with either a personal or a strong family history of cancer, you need to be very thoughtful as you contemplate treatment.

Your Decision:

While I am excited about the results I’ve seen with this treatment, this is NOT yet a widely accepted therapy.

You must feel comfortable with your decision for treatment, balancing your potential benefits against the risk of treatment, including the chance that it may not meet your expectation. More than one treatment may be needed. While I hope for a permanent result, you will continue to age, which means maintenance treatments will be necessary.
